Requirements:​

  1. Duly accomplished and signed application form INZ1017

  2. Two passport sized colored picture with white background

  3. Original passport (must be 6 months valid beyond intended stay)

  4. Original old passport if you have visa of the following (used within the last 5 years): Australia, Canada, Chile, Japan, Korea, Ireland, Israel, Mexico, New Zealand, Turkey, United Kingdom, US and Schengen

  5. Original bank certificate

  6. Photocopy of passbook

  7. Photocopy of time deposit (if any)

  8. Photocopy of last 3 months bank statement

  9. Photocopy of last 3 months credit card statement

  10. Photocopy of land title certificate

  11. Photocopy of car registration OR/PR

  12. Photocopy of any property documents (if any)

  13. Photocopy of PSA marriage certificate (if traveling with spouse)

  14. Photocopy of PSA birth certificate (if traveling with children)

  15. Photocopy of PRC ID for professionals (Doctors, Engineers, Accountants etc.)

  16. Photocopy of IBP ID for (lawyers)

  17. Photocopy of alien certificate registration (ACR) for foreign passport

  18. Photocopy of immigration certificate of residence(ICR) for foreign passport)

  19. Daily travel itinerary

  20. Round trip flight booking confirmation

  21. Hotel booking confirmation

Notes and Other Requirements:

For Employees:​

  1. Original employment certificate stating your position, tenure of service and income with benefits

  2. Photocopy of approved leave of absence

  3. Photocopy of company ID

  4. Photocopy of latest company income tax return (ITR)

For Business Owner:​

  1. Photocopy of business permit or mayor’s permit

  2. Photocopy of DTI or SEC

  3. Photocopy of latest income tax return (ITR)

For Students and Minors:​

  1. Original school certificate

  2. Photocopy of school ID

  3. Photocopy of registration form

For Retiree​

  1. Photocopy of senior citizen ID

  2. Retirement certificate (if early retiree)
