  2. 2 pcs. ID Picture with white background (Passport size) with SIGNATURE OVER PRINTED NAME at the back

  3. Photocopy of Passport (at least 6 Months Valid)

  4. Photocopy of Old passport and visas (if applicable)

  5. Original Bank Certificate

  6. Photocopy of Passbook

  7. Photocopy of Time Deposit (If any)

  8. Photocopy of Last 3 Months Bank Statement

  9. Photocopy of Last 3 Months Credit Card Statement

  10. Photocopy of Land Title Certificate

  11. Photocopy of Car Registration OR/PR

  12. Photocopy of Any Property Documents (If any)

  13. Photocopy of PSA Marriage Certificate (If traveling with spouse)

  14. Photocopy of PSA Birth Certificate (If traveling with children)
